You
must come with:
- Your 'carnet de santé'
- All the documents needed by the surgeon or anaestetist (medical
tests, x-rays...)
- your identity card, passport or resident's permit;
- your social security card (carte vital and supporting paper) or
the documents listed below depending on your status.
- your subscription card to a mutual insurance company or proof
that your charges will be covered by them.
- and, depending on your status:
Injured at work: (certificate testifying injury at work) in triplicate
presented by your employer
War pensioner (art. 115): your pensioner's card
Retired: your pension entitlement
Children under the age of eighteen: it is imperative that all children
be accompanied by the father and mother or legal guardian ( with
proof of identity ) as they will be asked to give their written
permission to operate.
If you do not have a social security card, you must produce the
following paperwork:
Salaried worker: pay slip
Assuré volontaire: proof of payment of contributions
Jobseeker:
- proof of registration with the A.N.P.E. ( Agence National Pour
l'Emploi )
- proof of payments of unemployment benefits by ASSEDIC
Student over the age of sixteen:
- card of the person looking after them (father, mother, or legal
guardian)
- proof of school attendance.
Student:
- registration card
- proof of enrolment for the current year noting the payment of
contributions.
Foreigner:
- National of the Europeen Union: proof that charges will be covered
(E 112 form for a planned hospitalization E 111 for an emrgency
hospitalization)
- National of a country outside the European Union: proof that charges
will be covered by an organization in your own country, or by your
embassy.
